One must be very careful using both PDE5 inhibitors and medications commonly used to treat an enlarged prostate, alpha-blockers (for example, tamsulosin [Flomax], terazosin [Hytrin]). It is recommended that one be on a stable dose of the alpha-blocker prior to starting a PDE5 inhibitor and that one start on a low dose of the PDE5 inhibitor and increase as tolerated and needed to treat the erectile dysfunction. Similarly, if you are on a PDE5 inhibitor and your doctor recommends that you start an alpha-blocker for your prostate, you should start at a low dose and increase as tolerated and needed to treat your prostate symptoms.
“Quite often, oral medications may be the only treatment required and men can resume normal sexual life. If a patient does not respond to oral medications, penile prosthesis (implants) is a viable and long-term option especially in severe cases. While least-chosen, it is important to know penile implants are easy to use and men report a high satisfaction rate. Speaking to an andrologist or urologist is the best option, as they can explain the risks and benefits of each treatment.”
Kegel exercises, or pelvic exercises, have proven to be effective in addressing erectile dysfunction, and it should be used as the first-line of treatment. The ischiocavernosus and bulbocavernosus muscles in the pelvic area surround the penis and are active during an erection. The following exercises aim to strengthen these muscles.
“Perhaps some men will be more motivated to know that they can improve their erectile function, sexual health, and quality of life, rather than just being told that they can live longer,” says Gerbild.
This can be useful even if there is a medical cause, as it gives you a chance to talk about how you feel about having erectile dysfunction. If there is an underlying psychological reason for the difficulty, this is a safe place to explore what this might be and what you can do about it. In fact, it has an 85 percent success rate. Possible side effects include a burning sensation and priapism, an erection that lasts more than four hours and requires medical treatment.
