If you have a partner, involve them in treatment by discussing your sexual function with them. Include them in physical activity, even if it's starting with a home-based walking program.
Besides PDE5 inhibitors and among second-line therapies are VCDs which are clear plastic chambers placed over the penis, tightened against the lower abdomen with a mechanism to create a vacuum inside the chamber. This directs blood into the penis. If an adequate erection occurs inside the chamber, the patient slips a small constriction band off the end of the VCD and onto the base of the penis. An erection beyond 30 min is not recommended. These devices can be a bit cumbersome, but are very safe.40 .

As blood flows into the penis, the corpora cavernosa swell, and this swelling compresses the veins (blood vessels that drain the blood out of the penis) against the tunica albuginea. Compression of the veins prevents blood from leaving the penis. This creates a hard erection. When the amount of cGMP decreases by the action of a chemical called phosphodiesterase type 5 (PDE5), the muscles in the penis tighten, and the blood flow into the penis decreases. With less blood coming into the penis, the veins are not compressed, allowing blood to drain out of the penis, and the erection goes down. Tablets for erectile dysfunction are called phosphodiesterase-5 (PDE5) inhibitors such as sildenafil (Viagra, Silvasta, Vedafil), tadalafil (Cialis) and vardenafil (Levitra). These medicines usually enable an erection by relaxing the blood vessels in your penis, but only when you are stimulated or aroused. They are all similarly effective. Read more about PDE5 inhibitors and frequently asked questions about their use.
The vacuum device creates a vacuum to pull blood into the penis. Unlike a normal erection, the inflow of blood does not continue once the individual removes the vacuum device. The rubber band placed at the base of the penis constricts the penis to prevent the blood from leaving the penis. As there is no inflow or outflow of blood when the rubber band is in place, it is uncommon for the tip of the penis (the glans) to appear a little blue and the penis to be cooler. Once intercourse is completed, the individual removes the rubber band and the blood drains out of the penis.

When you're working out regularly, you develop more endurance. That's important for your sexual health, because having sex is in itself a workout. The Mayo Clinic compares a bout of sexual intercourse to climbing two or three flights of stairs. And a study by the National Institutes of Health says a half-hour of sexual activity can burn 125 calories for men and nearly 100 for women, similar to walking at a 3 mile-per-hour pace.

The doctor will perform a complete physical examination, to check for abnormalities in the penis, scrotum, and testicles. To check for heart disease the doctor will take your blood pressure, measure your heart rate, and order a blood test to check your level of cholesterol. Your doctor will do a digital rectal examination to feel the size, shape, and consistency of the prostate, and if necessary check the level of prostate-specific antigen (PSA) in your blood. To check for diabetes the doctor may order a test to measure the level of glucose in your blood. It could also be necessary to assess your testosterone levels in the morning. This is also done with a blood test.

Monitoring erections that occur during sleep (nocturnal penile tumescence) can help you and your doctor to understand if the erectile dysfunction is due to psychological or physical causes. The nocturnal penile tumescence test is a study to evaluate erections at night. Normally men have three to five erections per eight hours of sleep. The test can be performed at home or in a sleep lab. The most accurate way to perform the test involves a special device that is connected to two rings. The rings are placed around the penis, one at the tip of the penis and the other at the bottom (base) of the penis. The device records how many erections occur, how long they last, and how rigid they are. The test is limited in that it does not assess the ability to penetrate.

Sometimes, blood flow problems develop because of damage to the endothelium or penile smooth muscle – tissues in the penis that are important for normal erections. This damage may be the result of high blood pressure or smoking. It can also happen if a man has high cholesterol, triglyceride, or blood sugar levels.
The sexual health of a person is as important as their mental and physical health. But often, it gets ignored, because there is a certain form of stigma attached to sexual health conditions. This even stops people from seeking timely treatment, if at all. Dr Gautam Banga, consultant and andrologist at the Sunrise Hospital New Delhi, says in India, Erectile Dysfunction (ED) is a taboo, because people don’t see it as a medical disorder but as sexual incompetence. “It does not happen because the man is not interested in or is incapable of having sex, but there are medical reasons like diabetes, high cholesterol, high blood pressure and ischemic heart disease (IHD), depression etc., that leads to ED,” he tells indianexpress.com.
