First-line treatment is typically with the use of phosphodiesterase inhibitors, such as sildenafil (also known as Viagra). In the UK, Viagra is now available without a prescription, and can be obtained over the counter from pharmacists, once suitability has been assessed. A vacuum device, or pump, can also be useful especially if medications are not suitable. Vacuum pumps work by encouraging blood flow to the penis, resulting in an erection.
This can be useful even if there is a medical cause, as it gives you a chance to talk about how you feel about having erectile dysfunction. If there is an underlying psychological reason for the difficulty, this is a safe place to explore what this might be and what you can do about it. This is generally not available as a funded service and you would normally need to pay for this privately. If you have been sexually abused, then you may be able to access free counselling through ACC.
Unlike other treatments for erectile dysfunction, PDE5i medications requires sexual stimulation to function. Without stimulation, these medications will not provide any effect.
Recently, the US Food and Drug Administration (FDA) has issued a safety announcement regarding TRT. In part it reads ‘The benefit and safety of these medications have not been established. We are also requiring these manufacturers to add information to the labeling about a possible increased risk of heart attacks and strokes in patients taking testosterone.’37

Erectile dysfunction refers specifically to problems achieving or maintaining an erection. Other forms of male sexual dysfunction include poor libido and problems with ejaculation. Hormone secretion is controlled by the body’s internal clock, and sleep patterns likely help the body determine when to release certain hormones.

“There are many lifestyle changes that are erectile dysfunction remedies, but among them, exercise is the ED treatment that can have the greatest impact,” says Zachary R. Patients may take tadalafil as needed as with sildenafil, vardenafil, and avanafil or once a day. It is the only ED oral medication that patients can take on a daily basis.
