In general, PDE5i works successfully in about 65%-70% of all men with erectile dysfunction (impotence). The greater the degree of damage to the normal erection mechanism and severity of the ED, the lower the overall success rate. Men with diabetes and those with spinal cord injury reported between 50%-60% responding successfully to treatment with oral PDE5i medications. The lowest success rate has been in men who developed ED (impotence) after prostate cancer surgery (radical prostatectomy) for more advanced prostate cancer that required removal of both sets of nerves around the prostate. In men who did not have the nerves removed/damage, there is a better chance of response to PDE5 inhibitors.
Watts and coworkers, in their review article, make several points about this ED/CAD nexus. Endothelial dysfunction is present in both CVD and ED, and is linked through the NO mechanism. The authors note that PDE5 inhibitors improve endothelial function and have a salutary effect on both CVD and ED. Both ED and cardiac disease respond to modifications in lifestyle as well as pharmacologic manipulation. These authors also report that the presence of ED gives the clinician an opportunity to assess CVD and prevention as well.20

There have been rare reports of priapism (prolonged and painful erections lasting more than six hours) with the use of PDE5 inhibitors such as sildenafil, vardenafil, and tadalafil. Patients with blood cell diseases such as sickle cell anemia, leukemia, and multiple myeloma have higher than normal risks of developing priapism. Untreated priapism can cause injury to the penis and lead to permanent impotence. Therefore, if your erection lasts four hours, you should seek emergency medical care.
University of Utah Health offers shockwave therapy as an option for patients suffering from ED. Shockwave therapy is typically recommended for patients who do not respond well to medication or do not want more invasive treatments.
Exercise does more than improve penile blood flow, however. Staying fit keeps a man’s weight under control, may increase his testosterone levels, and boosts his confidence. It can also reduce depression and anxiety. These factors can affect erections as well as other aspects of sexual health.
Erectile dysfunction is the most spreading of men’s problems. Kegel exercises are the best option to treat erectile dysfunction. Kegel exercises do not involve any type of harmful chemicals. So, there are no side effects and suitable for all types of persons.
Risk of ED is higher if you have high blood pressure, diabetes, vascular disease, low testosterone, history of prostate surgery, or take certain medications. Stress and other psychological aspects can sometimes play a role.
