The pathogenesis of organic ED is related to dysfunction of the endothelium. Endothelial cells can become injured through a variety of mechanisms, most of which cause oxidative stress on the tissues. Many of these causes of oxidative stress are related to lifestyle issues which lead to hypertension, diabetes and dyslipidaemia (figure 1). Endothelial cell dysfunction results in reduction of endothelium-dependent vasorelaxation as well as increased adhesion of leukocytes to the endothelium. Endothelial cell injury then leads to a variety of sequelae, including ED, other types of vasoconstriction, atherosclerosis and thrombus formation.18

The vacuum device creates a vacuum to pull blood into the penis. Unlike a normal erection, the inflow of blood does not continue once the individual removes the vacuum device. The rubber band placed at the base of the penis constricts the penis to prevent the blood from leaving the penis. As there is no inflow or outflow of blood when the rubber band is in place, it is uncommon for the tip of the penis (the glans) to appear a little blue and the penis to be cooler. Once intercourse is completed, the individual removes the rubber band and the blood drains out of the penis.

The role of the endothelium in erectile function became clearer with the observation that the phosphodiesterase type 5 (PDE5) inhibitor, sildenafil, enhanced erectile function. Erection occurs with the release of nitric oxide (NO) from the vascular endothelial cells.17 The reduction in endothelial cell production of NO results in the negative impact on the smooth muscles in the corporal bodies and results in less relaxation of the smooth muscle cells with decrease in blood supply and resulting ED. A similar phenomenon is well known to impact the coronary arterial system resulting in CVD.
Once significant systemic diseases, such as diabetes or heart disease, are ruled out, an oral medication, such as Viagra, Levitra, or Cialis, may be prescribed. If these agents are successful, further testing is usually not warranted.
Up to three quarters of patients with erectile dysfunction develop atherosclerosis, obesity, cardiovascular problems, diabetes, and similar diseases.
It is important to remember that having sex can put additional strain on the heart and may increase your risks if you have a history of heart disease or have an existing heart problem. The most common side effects of erectile dysfunction treatments include headaches, a sudden feeling of heat in your upper body, indigestion, visual disturbances, dizziness or a stuffy nose. Side effects are usually mild to moderate and don’t last long. If you experience any side effects or perhaps an increase in the severity of a side effect, you should stop taking the medication.
While medications are a common first therapy, they tend to only work in about 50 percent of men with diabetes. Good blood sugar control can help slow the progression of ED and sexual dysfunction in men. Men with diabetes are more likely to move on to other treatment options, such as the: vacuum erection pump, penile injection therapy, and penile implants. ED and Medications
Surgery is also an option for some men with ED. During this procedure, doctors place penile implants in the penis. There are different types of procedures, depending on your goals and preferences.

Erectile dysfunction is a defined as recurring inability to achieve and maintain satisfactory erection for sexual intercourse associated with relevant life impairment. The underlying etiologies may be manifold and complex. Currently, vascular etiologies are highly prevalent especially amongst elderly men. Of special interest, especially venogenic causes are of increasing relevance. Therapeutic options comprise risk factor modification, pharmacotherapy, surgical treatment, and endovascular treatment. Especially endovascular treatment options have recently increased in popularity including transcatheter embolization procedures for veno-occlusive dysfunction.
